NEOC calls on Upolu residents to complete energy crisis impact form

The National Emergency Operation Centre (NEOC) has released official assessment forms for households, businesses, and organisations in Upolu affected by the recent electricity crisis.​

Residents are urged to complete the form to help the government assess damage to appliances and equipment, disruptions to services, and loss of income. The collected data will guide planning and targeted support.​


What the Form Covers

The form collects specific details about how the power crisis has affected you or your organisation. This includes:​

  • Household and Contact Details: Basic information to identify and communicate with you.​
  • Type of Impact: Details on damage to electrical appliances, equipment, or loss of perishable goods.​
  • Financial Loss: Information on income or revenue loss resulting from the disruption.​
  • Supporting Documents: Such as receipts, photos of damage, or bank statements to substantiate your claims.​
  • Confirmation: For household submissions, a signature from a community leader or authorised person.​

Note: Completing this form does not guarantee compensation, but it assists the government in prioritising those most affected and planning assistance accordingly.​


How to Access and Complete the Form

  1. Download the Form:
  2. Print the Form: After downloading, print out the form.​
  3. Fill in the Form: Manually complete all required sections with accurate information.​
  4. Attach Supporting Documents: Gather necessary documents such as photos of damaged items, receipts, or any relevant evidence.​
  5. Submit the Form: Choose one of the following submission methods:​
    • Email Submission:
      • Scan the completed form and all supporting documents.
      • Email the scanned copies to: [email protected]
    • In-Person Submission:
      • Deliver the completed form and supporting documents to the NEOC Building at Tuanaimato.
    • Alternative Submission Points:
      • Households: Submit through your District Office (Fono Fa’avae) or the Ministry of Women, Community and Social Development.
      • Businesses: Submit to the Ministry of Commerce, Industry and Labour (MCIL) or the Samoa Chamber of Commerce.

Submission Deadline

All forms must be submitted by Friday, 25 April 2025.​
